It all depends by what tactic and what instructions you already have. I usually play with 4-3-1-2, so what I do is take off a midfielder, put the AM in midfield and bring another striker up. It would create a couple more chances. You might as well change mentality to attack.

Take off 3 worst performers, press your full backs and midfield foward. Generally adopt a more attacking do or die attitude.

Í don´t recommend many substitutions - they mess up your game for 10-15 minutes (and then the game can be over). 1 substitution is enough.. and bringin one more AMC helps too.
